About the Role

The Family Support Practitioner will work with families to empower them and build their capacity to make effective change. This role involves outreach work, engaging with families in their homes, and developing strong partnerships. The practitioner will use a client-led approach, focusing on building family awareness and insight into their challenges, rather than identifying issues through a practitioner-led assessment.

Key Responsibilities

  • Meet performance targets as per our dashboard system.
  • Provide high-quality practice that aligns with our values and vision.
  • Develop and implement case management plans that meet operational requirements.
  • Engage with clients using a warm and assertive approach.
  • Facilitate client-led assessments and care plans to identify client goals.
  • Deliver targeted interventions and group meetings to achieve client goals.
  • Commit to quality management through the use of LogiQC systems and MobNet tasks.
  • Build productive relationships with colleagues and external partners.

Desired Skills and Experience

COVID-19 Requirement

  • Immunisation history showing two doses of COVID-19 vaccine encouraged.

Required

  • Current Bachelor's Degree in Social Work, Human Services, or related field, with 2 years' relevant experience.

OR

  • Current Diploma in Community Services, Social Work, or related field, with 3 years' experience.

AND

  • At least 1 year of experience in a case management role.
  • Demonstrated ability to work in a performance-driven environment.
  • Ability to build client agency and facilitate change empowerment.
  • Demonstrated knowledge of behavior change interventions.
  • Ability to work with whole-of-family, irrespective of primary client.
  • Experience with coordinating multiple services and integrating internal agency supports.
  • Knowledge of factors contributing to vulnerability, disengagement, and intergenerational trauma.
  • Experience with case management for high-risk and/or crisis clients.

Additional Requirements

  • Requirement to travel across the region, as per program location.
  • Flexibility to work outside core hours in meeting individual client needs.
  • Working with Children's Card (Blue Card) or be eligible to apply.
