- Provide recovery-oriented and trauma-informed mental health assessments and triage services
- Conduct comprehensive screening assessments across the lifespan
- Assess and manage risk, allocating follow-up care in line with Victorian Mental Health Triage Response categories
- Direct referrals to the most appropriate team or external provider
- Support after-hours bed management for acute admissions
About the Position:
- Deliver high-quality, evidence-based care for patients with acute and chronic mental health needs
- Collaborate within a multidisciplinary team to provide patient-centered care
- Support patients and families in decision-making, advocacy, and discharge planning
- Supervise and mentor nurses where required
- Contribute to clinical quality, safety, and improvement initiatives
- Communicate clearly and confidently, particularly when discussing risks and patient safety
