- Mental health assessments to identify individuals requiring support and determine the most suitable care pathway.
- Short-term interventions to prevent hospitalization and promote effective follow-up post-mental health admission.
- Collaborative care for consumers transferring between hospital and community settings.
We offer specialist services for diverse age groups, including young people, children, families, adults, and older individuals. Our team prioritizes community-based care, working closely with stakeholders to deliver tailored ongoing care and treatment.
Ideal Candidate- Recognized tertiary qualifications in a relevant field.
- Current registration with a relevant professional body.
- A valid driver's license.
- Demonstrated competencies in providing contemporary mental health practice.
