Job Summary: We are currently working with a well-respected community service organisation that provides quality, responsive, and compassionate care to their local communities. The organisation is seeking an experienced Scheduling Officer - Care Coordination Specialist to join their team on a full-time locum basis.
Key Responsibilities:
- Manage customer relations and deal with program-based services
- Coordinate care packages and schedules for clients
- Develop and maintain strong relationships with clients, families, and community members
- Provide excellent communication and negotiation skills to ensure seamless care coordination
- Utilise CARELINK to manage client information and schedules
- Work collaboratively with the care team to ensure high-quality care delivery
- Contribute to the development and implementation of care plans
- Ensure accurate and timely documentation of client information
Requirements:
- 3 years minimum experience as a scheduler/rosterer and proficiency in CARELINK
- Experience in managing customer relations and dealing with program-based services
- Solid understanding of Home Care Packages
- Excellent time management skills
- High level of motivation, proactiveness, and team-oriented approach
- Excellent communication and negotiation skills
- Demonstrated sound knowledge of documentation and computer skills
- Demonstrated understanding of client, family, and community needs
