Job Summary: We are currently working with a well-respected community service organisation that provides quality, responsive, and compassionate care to their local communities. The organisation is seeking an experienced Scheduling Officer - Care Coordination Specialist to join their team on a full-time locum basis.

Key Responsibilities:

  • Manage customer relations and deal with program-based services
  • Coordinate care packages and schedules for clients
  • Develop and maintain strong relationships with clients, families, and community members
  • Provide excellent communication and negotiation skills to ensure seamless care coordination
  • Utilise CARELINK to manage client information and schedules
  • Work collaboratively with the care team to ensure high-quality care delivery
  • Contribute to the development and implementation of care plans
  • Ensure accurate and timely documentation of client information

Requirements:

  • 3 years minimum experience as a scheduler/rosterer and proficiency in CARELINK
  • Experience in managing customer relations and dealing with program-based services
  • Solid understanding of Home Care Packages
  • Excellent time management skills
  • High level of motivation, proactiveness, and team-oriented approach
  • Excellent communication and negotiation skills
  • Demonstrated sound knowledge of documentation and computer skills
  • Demonstrated understanding of client, family, and community needs
