Caregiver Advocate

As a Community Palliative Care Registered Nurse, you will have the opportunity to make a meaningful difference in the lives of individuals and families affected by advanced life-limiting illnesses.
This role offers the chance to work within a multidisciplinary team to provide patient-centered palliative care in the community, including direct care to clients and their caregivers in the home environment. Collaboration with primary care, community-based services, specialist services, aged care facilities, and group homes is essential to ensure comprehensive care for patients.
Palliative Care Responsibilities:
- Palliative care assessment and symptom management
- On-going care planning and nursing care
Requirements:
- Current registration as a Registered Nurse with AHPRA
- Valid NSW Driver's Licence
- Working With Children Check (WWCC)
- Applicable vaccination requirements for a Category A role
