Job Title: Aboriginal Health Rural Generalist Transitional Nurse Practitioner

The Coffs Harbour Health Campus and Galambila Aboriginal Medical services are seeking an experienced Nurse Practitioner to fill a challenging and rewarding role within our multidisciplinary team.

About the Role:

  • Provide high levels of clinically focused care across a range of services, including emergency, inpatient, chronic and complex care, and primary care.
  • Work autonomously and collaboratively in an advanced and extended nursing clinical role, meeting the clinical demand within their defined and agreed scope of practice and formulary.
  • Contribute to clinical excellence through research, teaching, consulting, and providing leadership to the nursing profession.

Key Responsibilities:

  • Receive and make referrals to facilitate integrated care across disciplines or teams.
  • Develop and implement effective care plans for patients with chronic and complex health conditions.
  • Support the development of best practice emergency/inpatient care plans and coordination of discharge plans for identified high-risk patients.
  • Establish culturally safe partnerships to address priority population groups within the Coffs Clinical Network community.

Requirements:

  • Registered Nurse (Division 1) qualification.
  • Postgraduate qualifications in Nursing Practice or related field.
  • Extensive experience in acute and chronic care settings.
  • Strong leadership and communication skills.
