Job Description

The Aboriginal Health Nurse Practitioner/Transitional role is a site-specific model providing high levels of clinically focused care across emergency, inpatient, chronic and complex care services.

  • Provide holistic patient-centered care for acute and chronic presentations addressing diverse community needs.
  • Develop integrated care plans across disciplines contributing to clinical excellence through research, education, consulting and leadership within the nursing profession.
  • Work autonomously and collaboratively in an advanced nursing clinical role meeting defined scope of practice and formulary requirements.
  • Create culturally safe partnerships to address priority population groups including Aboriginal and Torres Strait Islander people, people with disabilities, homeless individuals and older adults.
  • Cultivate relationships with internal and external healthcare agencies facilitating referrals and coordination of care.

This role aligns with the nursing profession's values, knowledge theories and practices, offering innovative flexible healthcare delivery that complements other providers.
