Job Description

This role is centered around collaborating with Director of Clinical Services, Regional Director of Operations, Administrators, and Directors of Nursing within the assigned region. Your key responsibility will be to develop, monitor, audit, and modify each resident's care plan for their individual needs and goals.

  • Conduct thorough assessments to ensure residents receive high-quality care.
  • Work closely with interdisciplinary teams to drive performance improvement initiatives.
  • Identify centers requiring enhanced support and work towards implementing targeted interventions.
Requirements

Required skills include a Bachelor's degree in nursing and gerontological nursing experience. You must also hold a current Licensed Registered Nurse designation.

Position Requirements

Experience working as an MDS Nurse Consultant in Skilled Nursing Facilities is essential.
