We're seeking a skilled Home Care Physiotherapist to join our team in West Melbourne. As a physiotherapist, you'll work closely with clients in their own homes, providing tailored treatment plans and conducting comprehensive assessments.

The ideal candidate will have strong communication skills, be outcome-focused, and dedicated to delivering exceptional care. You'll work autonomously to prioritize tasks, maintain records, and provide patient reports.

In this role, you'll have the opportunity to make a real difference in the lives of our clients. You'll be part of a supportive team environment, working holistically with other allied health professionals to provide a multidisciplinary care plan.

Key Responsibilities:
  • Conduct comprehensive assessments and deliver tailored physiotherapy treatment plans for clients in their own home through home care packages/NDIS
  • Work holistically with other allied health professionals to provide a multidisciplinary care plan
  • Educate clients and their families to optimize their ability to live healthy lives within the community
  • Liaise with service providers to ensure effective communication

Requirements:

  • Bachelor of Physiotherapy (or equivalent)
  • Registration with AHPRA
  • Police Check Clearance
  • Excellent verbal and written communication skills
  • Strong time-management skills and attention to detail
