About the Role

This role is part of a goal-oriented program designed to assist eligible hospital inpatients to return to living at home safely by providing low-intensity therapies and assistance with activities of daily living for up to 12 weeks.

The Transition Care Program ensures that local program therapies, services, documentation, communication, and administrative requirements meet the needs of the client and relevant guidelines.

Ideal Candidate
  • A current NSW drivers licence.
  • Tertiary qualifications in Nursing or Allied Health (specifically Occupational Therapy, Physiotherapy, Speech Pathology, Social Work or Dietetics) registration with AHPRA or membership to appropriate professional association if required.
  • Experience in assessment, care planning, monitoring, and review of aged care clients.

Key Responsibilities
  • To ensure that local program therapies, services, documentation, communication, and administrative requirements meet the needs of the client.
  • To maintain accurate and timely records of client progress and outcomes.
  • To communicate effectively with clients, families, carers, and health professionals to achieve optimal outcomes.
