Job SummaryWe seek an experienced Aged Care Registered Nurse to provide clinical expertise and leadership in a Clinical Nurse Consultant Grade 2 role. The successful applicant will coordinate comprehensive hospital avoidance strategies, facilitating optimal care for people aged 70 and over within Residential Aged Care Facilities (RACFs) and the community.
About The Role- Coordinate comprehensive hospital avoidance strategies to provide optimal care for people aged 70 and over within RACFs and the community.
- Facilitate appropriate fast track to diagnostic, assessment and treatment to avoid presentation to hospital or returning the person to their residence in a timely manner.
- Provide effective short term case management for older people, building capacity within RACF staff through the promotion of evidence based practice and advanced care planning.
- Formulate and implement policy, guide practice development, engage in education and research activities.
- Registered with Authority to Practice with Australian Health Practitioner Regulation Agency.
- Minimum five years full time post graduate experience with 3 years full time equivalent in the specialty field.
- Approved post graduate qualification (minimum Grad Diploma/Certificate level).
- Demonstrated clinical leadership skills and ability to work collaboratively within a multidisciplinary framework.
- Temporary Full Time opportunity until 4 December 2026.
- Possibility of ongoing employment.
