Job Title: Community Support Worker

Join Barossa Hills Fleurieu Local Health Network as a Community Support Worker and make a difference in the lives of our clients. As a key member of our team, you will provide personal care and support services to individuals in their own homes, promoting independence and well-being.

About the Role

This is a casual position working over a 7-day roster, with a focus on providing high-quality care and support to clients in the Southern Fleurieu region. You will work autonomously in the community, utilizing a wellness and reablement approach to service delivery and meeting client-directed goals.

Key Responsibilities
  • Provide personal care and support services to clients, including showering, bathing, toileting, and grooming
  • Assist with household tasks, such as cleaning, laundry, and meal preparation
  • Transport clients to appointments and activities, and assist with therapy services
  • Work collaboratively with clients, families, and healthcare professionals to achieve client goals and outcomes
Requirements
  • Current driver's license and access to reliable transportation
  • Ability to work autonomously in the community and make decisions under limited direction
  • Excellent communication and interpersonal skills, with the ability to work with diverse client groups
  • Commitment to maintaining client confidentiality and upholding the values of the Barossa Hills Fleurieu Local Health Network
