Clinical Care Coordinator
Job Description:We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our multidisciplinary team. As a key member of our consortium, you will play a vital role in delivering high-quality evidence-based clinical care to children and youth.
Our ideal candidate will possess excellent communication and interpersonal skills, with the ability to engage with people from diverse backgrounds and advocate on their behalf. You will work closely with staff, clients, families, and friends to establish and maintain effective relationships.
The successful applicant will have a strong understanding of the theory underpinning evidence-based interventions and clinical practice, with demonstrated excellence in the practical application of such in the areas of mental health and alcohol and drug use.
Key responsibilities include:
- Providing high-quality evidence-based clinical care as a member of the multidisciplinary consortia team.
- Addressing service gaps for the 'missing middle' by enhancing accessibility and integrating primary mental health care, psychosocial support, and early intervention.
- Promoting holistic wellbeing by considering broader determinants of health, fostering collaboration among providers, and ensuring a sustainable, adaptable service responsive to community needs.
This is a part-time opportunity that has the flexibility to work 4 days per week or a 9 day fortnight. While the role is primarily based at the hub in Gympie, there is also the flexibility to work from home one day per week.
Requirements:To be successful in this role, you will need:
- Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist, OR AASW registration/eligibility as a Social Worker, OR Level 2 ACA registration/eligibility.
- Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice, with demonstrated excellence in the practical application of such in the areas of mental health and alcohol and drug use.
- Highly developed communication (including written) and interpersonal skills.
- The ability to engage with people, advocate on their behalf, and establish and maintain effective relationships with staff, clients, families & friends and stakeholders.
- Experience in the use of electronic client/customer management systems, ability to use a desktop computer, and experience with Microsoft Office applications.
- Eligibility to apply/currently hold a Blue Card for paid employment.
- NDIS worker screening clearance for paid employment.
