The Advanced Healthcare Provider is responsible for delivering high-quality patient care and leading the multidisciplinary team in a dynamic healthcare environment.
This role requires a strong clinical background, excellent communication skills, and the ability to work collaboratively with other healthcare professionals to deliver person-centered care.
- Provide direct clinical care to patients, including assessing their needs, developing individualized care plans, and implementing evidence-based interventions.
- Lead the multidisciplinary team in providing care that meets the physical, emotional, and social needs of patients.
- Coordinate and oversee the delivery of clinical services, ensuring that patients receive timely and effective care.
- Develop and maintain relationships with patients, families, and healthcare professionals to promote a culture of respect and dignity.
Key Responsibilities:
- Assess patient needs and develop individualized care plans.
- Implement evidence-based interventions to improve patient outcomes.
- Collaborate with other healthcare professionals to deliver comprehensive care.
- Communicate effectively with patients, families, and healthcare professionals.
Requirements:
- Hold tertiary qualifications in Nursing and be registered with AHPRA as a Registered Nurse/Enrolled Nurse.
- Demonstrate high levels of clinical competencies with excellent clinical analysis, interpretation, and planning skills.
- Able to lead and influence others.
- Relates to the aged care population with empathy, understanding, and respect.
- Sound understanding of care assessment and provision within the Aged and Health Care sectors.
- Ability to work flexibly within a multidisciplinary team environment.
