About the Role

You will collaborate with our Quality team to establish and maintain high standards of professional practice through effective wound and skin integrity project implementation. This includes contributing to policy development, audit processes, and evaluation procedures. Additionally, you will work closely with care home teams to complete holistic assessments and telehealth reviews for complex and chronic wounds.

  • Support a robust monitoring program to identify compliance risks and opportunities for improvement in clinical systems.
  • Develop, implement, and evaluate clinical policies that foster continuous quality improvement in patient care and services.
  • Analyse data to identify areas for enhancing clinical system efficiency and effectiveness.
  • Evaluate clinical assessment, care planning, and documentation processes to ensure alignment with evidence-based practices.
  • Manage a designated portfolio, including Wound Consultant, Infection Control/IPC Lead, Insights and Analytics, and Governance.
Your Responsibilities
  • Quality Improvement:
    • Resolve system issues and tickets, providing guidance on our Quality Management System and Clinical systems.
    • Collaborate with the broader Quality team and Care Homes to develop and deliver contemporary clinical practice, particularly in skin integrity and wound management.
    • Sustain an ongoing program for continuous improvement at the care home level, focusing on Plan for Continuous Improvement (PCI).
  • Relationship Building:
    • Foster trusted relationships with key stakeholders and seek regular feedback.
    • Develop strong connections with vendors, external stakeholders, prescribers, allied health professionals, and pharmacies.
    • Promote a collaborative team spirit to achieve business outcomes across our organisation.
  • Change Management:
    • Implement digital solutions based on clinical expertise and product knowledge, actively managing issues and lessons learned from previous projects.
    • Support projects by conducting analysis and impact assessments, including testing and training.
    • Lead the design, implementation, and delivery of digital clinical systems and strategies.
    • Promote awareness and adoption of digital health products, services, and systems within our organisation.
    • Work with solution vendors to influence roadmaps, release management, and deployment of change into our care homes.
    • Promote nurses' leadership in designing, implementing, and evaluating digital health technologies supporting nursing practice.
  • Community:
    • Contribute to project delivery best practices and participate in Communities of Practice within our organisation.
Requirements
  • Bachelor of Nursing degree;
  • Current AHPRA registration;
  • Demonstrated experience in aged care sectors or wound management roles;
  • Experience working with clinical systems;
  • Research skills;
  • High level of Digital Literacy;
  • Substantial knowledge of aged care industry;
  • Substantial knowledge of Aged Care regulatory requirements;
  • Planning and organisational skills;
  • Coaching and leadership skills;
  • Team leadership skills and ability to work interdependently in a challenging environment;
  • Strong communication and relationship management skills;
  • Project and Change Management skills;
Desirable Qualifications
  • Relevant postgraduate qualifications in wound management or related field;
  • Experience in delivering educational presentations to key consumers;
