Key Responsibilities

  • Conduct comprehensive mental health assessments and develop individualized treatment plans in collaboration with consumers and their caregivers;>
  • Provide education and support to consumers and their families on mental health issues, treatment options, and coping strategies;>
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and delivery;>
  • Maintain accurate and timely records of consumer interactions, progress notes, and outcome measures;>
  • Participate in regular team meetings and contribute to quality improvement initiatives.

Requirements

To be successful in this role, you will require:

  • ">
  • A degree in Social Work, Occupational Therapy, or Psychology from an accredited institution;>
  • Current registration with AHPRA (Australian Health Practitioner Regulation Agency) or eligibility for membership with the Australian Association of Social Workers;>
  • Demonstrated experience working in mental health settings, preferably in community-based roles;>
  • Strong communication and interpersonal skills, with ability to build rapport with diverse populations;>
  • Excellent time management and organizational skills, with capacity to prioritize tasks and manage competing demands.
