We are seeking an exceptional individual to lead our care team and make a meaningful difference in the lives of our residents.

About the Role

As a Care Manager, you will be responsible for overseeing daily clinical operations, supporting your team, and making decisions that impact the lives of our residents every day.

  • You will lead with purpose - ensuring our residents receive the best care available, and supporting your team to deliver excellent results.
  • Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
  • Review clinical incidents, complaints and changes, providing solutions to reduce any risks.
  • Lead compliance with assessments and evaluations.
  • Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
  • Foster a strong team environment.
  • Prepare and conduct audits.
  • Provide insights and innovative solutions for continuous improvement.
  • Oversee medication management in accordance with resident needs and legislative requirements.
  • Liaise with allied health, medical practitioners and other services to provide optimal resident care.
Requirements
  • Bachelor of Nursing with current registration is essential.
  • Post-graduate qualification in aged care highly regarded.
  • Strong and effective leadership, communication and interpersonal skills.
  • A person-centred care mindset.
  • A passion for nurturing a strong team environment.
  • Ingrained empathy for our residents.
  • Proven track record in delivering quality clinical care within an aged care or acute setting.
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
  • An understanding of Resident Rights, Accreditation Standards and Outcomes.
