Care Support Specialist

We are seeking a skilled and compassionate individual to join our team as a Care Support Specialist. This is an exciting opportunity to make a meaningful difference in the lives of people experiencing homelessness.

About the Role:

The successful candidate will provide vital care support to individuals, helping them stabilize their accommodation and transition away from homelessness. You will work collaboratively with clients, government agencies, and other service providers to ensure a holistic approach to support.

Your Key Responsibilities:

  • Engage with individuals experiencing homelessness or at risk of homelessness
  • Conduct initial intakes and assessments, providing crisis intervention when necessary
  • Refer clients to external services and connect them to life skills and educational programs
  • Advocate on behalf of clients with government departments, courts, and other service providers
  • Conduct home visits for clients in temporary or transitional accommodation
  • Explore opportunities to help individuals reach their full potential
  • Build collaborative partnerships with key stakeholders and services
  • Provide client support with a respectful, non-judgmental, and inclusive attitude, while respecting client confidentiality
  • Create a safe and welcoming environment for clients accessing the service & attend to clients' primary needs
  • Develop case plans with clients, provide ongoing support, and monitor, review, and follow up on the case plan
  • Maintain client records and case notes in a professional manner
  • Manage client flow and building safety
  • Handle phone calls, mail, and correspondence

About You:

To be successful in this role, you will require:

A minimum TAFE Certificate IV or currently undertaking tertiary study, in welfare or related fieldPrevious experience providing support to marginalized and vulnerable people, particularly those experiencing homelessnessDemonstrated knowledge of issues relating to homelessness, including family breakdown, mental illness, drug, alcohol, and gambling addictionsDemonstrated awareness of appropriate local community support services for homeless clients, including government agencies and NGOs, and the capacity to build and maintain strong relationships with such service providersExcellent written and oral communication skillsSound computer skillsAwareness and understanding of the Work Health and Safety legislation and a commitment to promoting safe work practicesAbility to use sound judgment and act appropriately in crisis and emergency situationsA NSW driver's license
