Join DPV Health as a Clinical Nurse Coordinator

DPV Health is expanding the Pathway to Good Health service for vulnerable children and young people in Out of Home Care, in partnership with the Department of Health and DFFH Child Protection. We are seeking a skilled Clinical Nurse Coordinator to support our multidisciplinary team in providing comprehensive health assessments and care coordination.

The successful candidate will be responsible for referral management, triage, and health history collation, working closely with the child's care team to identify unmet health needs. They will also provide clinical expertise and trauma-informed care to contribute to holistic Health Management Plans.

About the Role:
  • Act as an interface between the Child Protection system and health services, ensuring cross-sector collaboration and navigation.
  • Conduct secondary consultation with DFFH Child Protection North Division and other partners to provide clinical triage and identification of unmet health needs.
  • Gather health and social information to collate a comprehensive health history and communicate clinical information effectively.
  • Identify and navigate barriers to service engagement by offering a flexible model of care.
  • Collaborate with the multidisciplinary team to conduct comprehensive health assessments and contribute to Health Management Plans.

We are looking for a highly skilled and compassionate nurse who can work autonomously within their scope of practice, while meeting agreed timelines and collaborating with a range of stakeholders to improve health outcomes.

About You:
  • Current APHRA Registration as a Division 1 Registered Nurse.
  • Paediatric nursing experience of at least three years post-registration and postgraduate qualification in a speciality area.
  • Experience working with vulnerable children and adolescents and culturally and linguistically diverse clients.
  • Well-developed paediatric and adolescent clinical assessment skills and a sound understanding of child development, attachment theory, and the social determinants of health.

We offer a competitive salary and benefits package, including a $95,000 - $110,000 per annum salary range, depending on experience. Successful candidates will also be required to undergo a National Police Check, International Police Check (if applicable), NDIS clearance, and hold a valid WWCC.
