
Support Coordinator

Key Responsibilities:
Work collaboratively with participants to understand their NDIS plans and goals
Assist participants to connect with informal, community, mainstream, and funded supports
Coordinate services and supports across multiple providers to ensure a holistic approach to service delivery
Monitor the effectiveness of supports and outcomes, and assist participants with plan reviews
Resolve points of crisis and assist in reducing barriers to plan implementation
Maintain accurate case notes, service records, and progress reports in line with NDIS and organisational requirements
Promote participant choice and control in all decision-making
Liaise with NDIA, support providers, mainstream services, and informal support networks as required
Provide culturally appropriate and person-centred support tailored to each participant's needs
Selection Criteria:
Essential:
Relevant qualification in Social Work, Psychology, Community Services, Disability, or related field
Minimum of two (2) years' experience working as a Support Coordinator or in a similar NDIS coordination role
Demonstrated knowledge of the NDIS and the role of Support Coordination
Strong understanding of disability rights, person-centred practice, and trauma-informed care
Excellent interpersonal, communication, and negotiation skills
Ability to work independently, manage time effectively, and maintain professional boundaries
Proficiency in using client management systems and Microsoft Office suite
Current driver's licence and reliable vehicle
Current Working with Children Check (if required)
NDIS Worker Screening Check
Current First Aid & CPR (or willingness to obtain)
Desirable:
Experience in case management, care coordination, or similar roles
Lived experience of disability or experience working with people with complex needs
Understanding of psychosocial disability and mental health supports
Key Performance Indicators (KPIs):
Participant satisfaction and engagement levels
Timely implementation and coordination of services in line with NDIS goals
Quality and accuracy of documentation and reporting
Responsiveness to participant needs and crisis management
Contribution to team goals and organisational values
Work Conditions:
Casual position with flexible hours based on participant needs and availability of work
Combination of home-based, and community work
Travel within the region may be required
