Who We Are & Why We’re Different
Bridges Health & Community Care Ltd. (BHCC) is a purpose-driven, community-based organisation delivering life-changing programs across regional and rural Queensland. Since 1997, we’ve helped thousands of people navigate mental illness, addiction, disadvantage and complex needs – moving from isolation to connection, from stigma to purpose.
We offer a wide range of services, including:
- Mental health, drug & alcohol, and counselling support
- Youth, justice and family-focused programs
- NDIS services and disability supports
- Innovative initiatives like BridgeWorx and the Mind-Life project
Our work is grounded in recovery-oriented practice, “No Wrong Door” principles, and a deep commitment to lived experience, cultural respect and inclusion. We are proud to be launching the Staying on Track program — and we’re looking for passionate people to help lead the way.

Multiple Opportunities – One Shared Purpose
We’re recruiting for three full-time positions under the Staying on Track (SoT) program, funded by the QLD Department of Youth Justice and Victim Support as part of the Making Queensland Safe initiative.
This program supports young people aged 10–17 who are exiting detention, helping them safely reintegrate into their families and communities, with wraparound support for education, training, employment, cultural identity, and resilience.
Team Leader – People & Culture
Family Support Practitioners (1.5 FTE)
Youth Mentors (General & Cultural, 4 - 5 FTE)
Please Note: Mentors provide continuity from detention through release, offering crucial support - especially during the high-risk first 72 hours post-release.
Shared expectations for all roles:
You bring the values, flexibility, and initiative that young people need most. You’ll thrive in this work if you:
- Have strong communication and problem-solving skills
- Stay calm, grounded, and focused in dynamic situations
- Bring cultural awareness and a deep respect for lived experience
- Are flexible, collaborative, and ready to travel regionally as needed
- Believe in building safety, trust and growth for every client
Pre-employment Requirements (All Roles)
All team members must hold (or be willing to obtain before commencement):
- Working with Children Check (Blue Card)
- NDIS Worker Screening Card
- National Criminal History Check
- Queensland Open Driver’s Licence
Why You’ll Love Working With BHCC
You’ll join a program that’s still being shaped, which means your voice and impact will matter from day one. BHCC offers:
- Competitive salaries + salary packaging of up to $18,550 per annum
- Flexible working arrangements
- Relocation assistance to help you settle
- Access to an Employee Assistance Program (EAP)
- Travel reimbursement + Access to fleet vehicles
- An additional 5 days of Health and Wellbeing Leave annually
- Ongoing training and professional development
- A culture that supports lived experience and cultural leadership
- A chance to do work that truly changes lives
Please note: After-hours work, regional travel, and some weekend availability may be required for these roles.
Additional Info:
- Start date: We’re recruiting now, with roles commencing ASAP or by October 2025.
- All roles are based in Hervey Bay or Bundaberg, with travel to Gympie and North Burnett as needed.
- Full induction, onboarding, and cultural supervision will be provided to all successful candidates.
