General Practice Alliance (GPA) are looking for a Care Coordinator/Phlebotomist to join our team on a full-time basis.
This is an exciting new role combining safe, compassionate blood sample collection with proactive care coordination. You'll be working closely with patients to ensure they feel supported, connected to the right services, and confident in managing their health and wellbeing.
Please note: We are unable to offer visa sponsorship for this role.
Main duties of the job
As our Care Coordinator/Phlebotomist, you/'ll spend time both in practices and working from home, carrying out phlebotomy while also reviewing patient needs, triaging referrals, and helping people access the right support.
We/'re looking for someone compassionate, organised, and confident, who enjoys connecting with patients and supporting them through their health journey. Phlebotomy experience is desirable, but if youre not already trained, we will provide full training. What matters just as much is your dedication to providing personalised care and helping patients feel supported every step of the way.

Job responsibilities
Perform safe and compassionate blood sample collection in line withclinical procedures.
Accurately label, store, and dispatch blood samples to appropriatelaboratories.
Document sample collection and care coordination activities in patientrecords.
Review patient notes to identify unmet biopsychosocial needs oropportunities for further support.
Engage with patients to coordinate follow-up actions.
Act as a central point of contact for patients, promoting continuity ofcare.
Support patients in preparing for and following up on clinicalconversations.
Proactively identify frequent attenders with complex needs andcoordinate their care.
Signpost and navigate patients to relevant health, social, and communitysupport services.
Triage incoming referrals and direct patients to appropriatepersonalised care services based on individual needs and preferences.
Escalate anomalies or significant patient concerns to clinical staff.
Person Specification
Qualifications
- Educated to GCSE Level (5 GCSEs at grade C or equivalent inc. English Language and Maths).
Job Circumstances
- Reliable transportation to facilitate a hybrid work arrangement and occasional travel for mandatory meetings as required by the position.
- Possess the legal right to work in the UK and ability to provide appropriate documentation for verification.
Experience
- Experience of working within primary care, community or hospital settings.
- Trained and competent in phlebotomy, with experience in blood sample collection OR willing to complete phlebotomy training shortly after appointment.
- Experience of working directly in a care co-ordinator role.
Skills and Knowledge
- Confidently use databases to review patient records, identify needs, update information, and coordinate tasks, with a commitment to confidentiality, safeguarding, and patient safety.
- Ability to listen, empathise with people and provide person-centred support in a non-judgemental way.
- Organised and able to manage multiple tasks and priorities effectively.
- Knowledge of IT systems, including ability to use word processing skills, emails, and the internet.
- Basic knowledge of long-term conditions and their impact on physical, emotional and social needs.
