Delivered Meaningful Life Programs to our residents Leadership role offering Full time hours, working Monday - Friday. Located in Wahroonga Tallwoods Care Community.

Responsibilities

- Monday - Friday, 9am - 5pm
- Lead the Meaningful Life program in the Care Community
- Develop opportunities for meaningful engagement, purposeful participation, social events and activities for all residents
- Drive Opal HealthCare initiatives including Meaningful Mates and Helping Hands
- Curate programs to support residents’ physical, emotional and spiritual wellbeing and overall quality of life
- Develop resident care plans and complete key lifestyle assessments
- Work together with clinical teams and allied health professions provide a tailored approach for each resident to aid their mobility and independence
- Coach care team members to deliver meaningful engagements between like-minded residents
- Liaise with residents to develop activities and engagements that meet their cultural, spiritual, social and physical needs and preferences
- Engage with residents’ families to encourage visitation and socialisation
- Demonstrate Person Centred Care principles approach
