About CHATS
CHATS plays a vital role in bridging hospital and community care, supporting consumers and their carers through transition from acute settings to home or residential care.
We deliver a wide range of services designed to assess, re-ablement, and maintain individuals’ capacity to successfully remain in their community — including:

- Specialised and intensive rehabilitation
- Low-intensity and maintenance therapy
- Basic support and re-ablement programs

You’ll work closely with a trans-disciplinary team that includes nursing, occupational therapy, physiotherapy, social work, psychology, speech pathology, dietetics, continence care, and personal care assistants — all united by one goal: helping people live their best lives.

What You’ll Bring

- Relevant tertiary qualification and registration (as applicable)
- Experience working with adult or aged care consumers (preferred)
- Excellent communication and collaboration skills
- A passion for empowering others through goal-directed, person-centred care

Requirements

- Current Aged Care National Police Certificate (mandatory)
- Current driver’s licence and reliable vehicle (highly regarded)
