About the role and where you will be working:
To provide excellence in coordinating services by applying a wellness and reablement approach for clients to achieve their goals and improve their health and wellbeing outcomes. To work collaboratively with clients, internal and external stakeholders to assess, plan and facilitate the delivery of care to clients considering their assessed care needs strengths and goals.
We’ll also be asking you to:
Effectively investigate and resolve clients concerns and complaints in a proactive manner in relation to frontline service delivery staff in a collaborative model with all levels of Home Care management.
Complete all necessary Annual Performance Reviews and staff management responsibilities.
Provide data and reports as required for programs, and to assist with workforce development
Ensure policies, procedures and work systems are maintained and all quality and accreditation guidelines are met.
Complete any necessary documentation to support the delivery of care.
About you:
You’re a compassionate and proactive leader with experience coordinating care services and supporting clients to achieve their goals. You communicate clearly, build strong relationships, and approach challenges with optimism and integrity. With sound knowledge of home care practices and a focus on continuous improvement, you lead by example and foster teamwork, accountability, and respect.
To be considered for this role, you’ll need:
Demonstrated customer service orientation and continuous improvement focus.
Demonstrated computer literacy (Microsoft Office, Outlook and Excel) minimum intermediate level of competency.
Up to date with home care legislation and the impact of recent changes on care delivery
Intermediate MS Office skills and experience
