Restorative Care Coordinator

About the role and responsibilities
The Restorative Care Coordinator works in partnership with older people, their families, carers, and key stakeholders to plan, coordinate and oversee short‑term, goal‑oriented interventions under the Restorative Care Pathway, ensuring services are evidence‑based, person‑centred and designed to maximise functional independence, wellbeing and quality of life.
- Conduct timely intake and eligibility assessments in line with Restorative Care Guidelines.
- Develop and implement individualised Restorative Care Plans that reflect assessed needs, client goals, cultural and language preferences and desired level of participation.
- Liaise with external providers, GPs, hospitals and community services to support integrated care pathways.
- Monitor expenditure and ensure services remain cost‑effective and within program parameters.
- Support clients and their families to prepare for transition and ensure they are connected to appropriate supports.
Qualifications
- Relevant qualification in Nursing, Allied Health or Community/Aged Care (minimum Certificate IV in Allied Health Assistance or equivalent experience).
- Demonstrated experience in aged care, community health or restorative/reablement service delivery.
- Knowledge of Support at Home reforms, Restorative Care Guidelines and aged care regulatory frameworks.
- Strong skills in assessment, care planning, case management and goal‑directed support.
- Excellent interpersonal and communication skills, with the ability to engage clients, carers and stakeholders.
- Proven organisational and time‑management skills, including the ability to manage competing priorities.
