Remote Monitoring Clinical Delivery Lead
Be part of a pioneering new service that's transforming how we care for patients across Sussex. Remote Monitoring is designed to support patients at home, prevent unnecessary hospital admissions, and deliver compassionate, timely care through innovative virtual solutions.
As a Remote Monitoring Clinical Delivery Practitioner within Community Care, you'll oversee utilisation of remote monitoring licenses and equipment across community services. You will support the development of future clinical pathways and provide clinical advice and guidance to internal and external stakeholders including clinicians, care homes, acute trusts and primary care, whilst supporting continuous evaluation and regular reporting of metrics.
While patient monitoring will take place remotely, this role is not remote. The base is negotiable but will be within an SCFT site, working alongside community care colleagues. The role will require travel across the SCFT footprint to provide clinical leadership, support implementation of the remote monitoring licences, and work directly with teams and the remote monitoring hub to embed the service.

What We're Looking For
We're seeking a registeredNurse or Paramedic (NMC/HCPC)with:
Excellent and proven leadership skills
A positive and patient-centred approach
- Experience in leading and supporting colleagues with new and developing initiatives
- Understanding of the benefits of digital health care and remote monitoring
- Knowledge of frailty, heart failure, COPD, and diabetes
- Excellent communication, documentation, and IT skills

Person Specification
Qualifications
- Registered General Nurse or Paramedic - NMC or HCPC
- Evidence of post registration experience
- Educated to degree level
- Postgraduate qualification in health-related subject
Experience
- Planning & organisation
- Evidence of well-developed critical decision-making skills
- Experience of quality improvement/audit
- Understanding of the Health Care Agenda
- Demonstrates commitment to and recent evidence of further education and professional development
- Ability to deal with a wide range of people at all levels including clients, carers, therapists, health and social care colleagues and other professionals
- Demonstrates a positive attitude towards change
- Experience and ability to influence, negotiate and persuade, using own initiative
- Knowledge of clinical governance
- Experience of working with digital health/remote monitoring systems
- Understanding of multi-disciplinary working in the care of adults
- Understanding of the Single Assessment Process. NHS experience and knowledge. staff

- Computer Literate
- Demonstration of clear, accurate and precise documentation

- Knowledge of SystemOne
Other requirements
- Ability to work flexibly within a shift pattern
- Ability to travel to various work bases.
