About the role

As a Case Worker with our Reconnect Program, you’ll provide tailored outreach support to young people and their families to prevent homelessness. With a trauma-informed, person-centred and strengths-based approach, you will:

- Build trusting relationships and deliver counselling, mediation, advocacy and practical support
- Develop assessments and action plans that help clients overcome barriers and work towards stability and wellbeing
- Connect young people and families with support services, education, housing and community networks
- Promote resilience, independence and social inclusion through early intervention and prevention strategies

You’ll be part of a collaborative and supportive team, working to empower young people.

About you

You’re passionate about walking alongside young people and families, helping them navigate challenges and build on their strengths to prevent homelessness. You thrive in dynamic environments and are confident engaging with people from diverse backgrounds, always bringing empathy, professionalism and resilience.

Requirements

- A relevant bachelor’s degree (Human Services, Social Work, Psychology, Counselling) or significant experience in case work
- Proven experience supporting people at risk or facing complex challenges such as housing instability, mental health or family conflict
- Excellent communication, active listening and relationship-building skills, with the ability to engage with young people and families in meaningful ways
- Cultural awareness and sensitivity, with experience working across diverse communities
- Confidence using MS Office and maintaining accurate, timely case records
- A current WA WWCC
- Driver;s License
- National Police Clearance (obtained within the last 6 months)
