Overview

We have a permanent part-time opportunity for a skilled, enthusiastic and dedicated Commonwealth Home Support / DVA / VHC Care Case Manager to join our Home Care Team in Launceston with regular travel to Shearwater.

Responsibilities

- Provide day to day management of a client case load, care planning and provision of consumer directed services to clients

- Make a real difference to our consumers lives

- Enable our consumers to remain in our own homes

What you bring to the role

- Relevant experience in case management, community services, aged care or equivalent (Essential)

- Knowledge of CHSP and VHC funding (Essential)

- Experience in service delivery to clients living in the community

- High level planning and organisational skills

- A desire to create a positive experience for our consumers
