About the role

We currently have a permanent part-time opportunity for a skilled, enthusiastic and dedicated Commonwealth Home Support / DVA / VHC Care Case Manager to join our Home Care Team in Launceston willing to travel to Shearwater on a regular basis.

As a Commonwealth Home Support / DVA / VHC Care Case Manager, you will have an opportunity to:

  • Provide day to day management of a client case load, care planning and provision of consumer directed services to clients
  • Make a real difference to our consumers lives
  • Enable our consumers to remain in our own homes

What you bring to the role

  • Relevant experience in case management, community services, aged care or equivalent (Essential)
  • Knowledge of CHSP and VHC funding (Essential)
  • Experience in service delivery to clients living in the community
  • High level planning and organisational skills
  • Well-developed communication skills
  • A desire to create a positive experience for our consumers
  • A current unrestricted drivers license (Essential)
