- Coordinate the establishment of support by the relevant team member for new participants/workers on a case by case basis. This includes nurturing relationships with participants/workers and their family during their application to the scheme by providing information/support about the processes and expectations of the scheme, linking families to appropriate community and mainstream supports, building strong collaborative relationships with hospitals and referral partners and ensuring a smooth transition to an appropriate support person.

- Foster a team culture to drive high performance and support the integration of roles within the team to ensure collaborative, evidence based decision-making practices are applied to deliver optimal outcomes to participants/workers and their families, ensuring adherence to protocols.

- Contribute to service improvement strategies and a learning culture by actively collaborating across Care and Community in the design and implementation of internal quality assurance, and continuous improvement systems to drive quality and consistency in decision making.
