As a Support Coordinator, you will embody our core values such as Customer Always and Everyone Matters, providing participants with access to supports to live independently and achieve their goals. Your day-to-day activities may include setting up service agreements, preparing plan reviews, conducting risk assessments, attending care team meetings, and providing support when issues arise.
You will have the flexibility to plan your day, focusing on supporting your customers.
We will equip you for success with specialised training, regular coaching, and an experienced team to support you.
What you will bring to the role:
- Empathy, resilience, excellent time management, and a commitment to a people-focused culture.
- Proven success in meeting productivity and activity targets.
- Experience in Portfolio and Stakeholder Management.
- A strong work ethic and resilience.
- A current Australian driver’s license, a well-maintained vehicle meeting safety standards, and comprehensive car insurance.
- NDIS Worker Screening Check and Working with Children/Vulnerable People checks as per legislation.
