This role is offered as a **Fixed Term Full-time contract up until November****2027 with the potential to an ongoing, based out of Hobart or Launceston.**Due to increasing demand, we are expanding our services to support more people living with dementia. **Benefits** Salary packaging; $15,900 annually for living expenses, plus up to $2,650 each year for entertainment benefits. Leave benefits – purchased additional annual leave**Responsibilities** Undertake a range of assessments that help to understand the holistic experience of the person and their care network. Work on other projects and assignments as required and in line with areas of skill, expertise and knowledge.Experience in Australian health, hospitals or aged care (community or residential).Excellent problem solving, analytical and decision-making skills. Be relatable and able to share knowledge in a way that support change at all levels Able to prioritise multiple tasks and meet deadlines.Great written communication and report-writing skills.
