Job Description

The Support Coordinator plays an integral role for NDIS participants in interpreting their NDIS plan, assisting them to establish the supports in their plan, and developing their capacity over time to coordinate and manage their own supports.

Primary Role:

- Assist NDIS participants to:

- Implement their plan and increase their capacity to manage/direct their own support
- Have greater opportunities to explore and connect with their community and alternative support options
- Strengthen their informal support networks
- Maximize the value for money they receive from their supports
- Access and use the NDIS Participant Portal (my place)

• Effectively communicate and maintain relationships with the NDIA. Maintain a company KPI of bringing in 1 client per week to Point Care for Support Coordination (thereby increasing their caseload).

Accountability:

- Action referrals in a timely manner
- Support the NDIS participant to:
- Assess a number of mainstream, community, informal, and provider options
- Select preferred options or providers
- Negotiate services to be provided and their prices
- Develop service agreements and create service bookings with preferred providers
- Arrange for any assessments required to determine the nature and type of funding required
- Determine the budget for each support type and advise relevant plan managers of the breakdown of funds
- Liaise with plan managers to establish appropriate claim categories and attribute the correct amount of funds
- Link to mainstream or community services (housing, education, transport, health)
- Strengthen and enhance participants' capacity to coordinate, self-direct, and participate in the community, including providing assistance to resolve problems, understand responsibilities, and change or end support agreements
- Be the key contact for service issues, complaints, major changes, and plan reviews
- Assist participants to prepare for their plan review by assessing goal achievement and value for money
- Identify solutions to problems experienced in implementing the plan
- Consider new goals and actions for achieving goals, including exploring housing options and life transition planning
- Maintain confidentiality of customers, families, carers, and the organization
- Network internally and externally to assist individuals in achieving personal outcomes
- Manage conflicts of interest according to NDIA’s Terms of Business
- Provide NDIA with reports on goals, outcomes, and success indicators as per reporting schedule
- Continuously improve Point Care's reputation through practice, business development, and innovation

Requirements

- A commitment to the Code of Conduct and Point Care policies and procedures
- Excellent interpersonal and active listening skills
- Excellent written and verbal communication skills
- Ability to adapt communication style to meet people's needs
- Conflict resolution skills
- High numeracy and literacy levels
- Strong administrative skills including time management and task coordination
- Proficiency with systems and technology
- Ability to work independently
- Ability to build and maintain community connections and relationships with service providers, individuals, and families/carers
- Understanding of the NDIS Price Guide, legislation, rules, and the role of mainstream services, including child safety and advocacy
