Grade 4 (YW15) Case Manager role in Hospital in the Home & Virtual Care
- Support patients and families to access care from home as part of an innovative team
- Full time or part time available, permanent roles
- Located primarily at Austin Campus, with attendance to other Austin campuses as required.
- $54.63/hr base + salary packaging + super (pro-rata for part time)

**About **Hospital in the Home & Virtual Care**
Hospital in the Home & Virtual Care facilitates the shift of traditional acute admitted care to the patient’s own home without compromising patient care. This care may take place via in-home nursing and support services, via video telehealth or via admission to the Hospital in the Home - Virtual Care, where higher acuity care can be provided in part through the utilisation of wearable technology.
**About the role**
Hospital in the Home (HITH) & Virtual Care (HITH-VC) Case manager will proactively manage the transfer of patients from a traditional inpatient model to receiving high quality, acute care in the patient’s own home. They will manage the needs of patients in a collaborative fashion to ensure high patient satisfaction, while facilitating hospital flow and safe and effective discharge planning to community services post HITH admission. Care is provided by the most effective means - be that via in person nursing care, via Telehealth, virtual ward admission (including the use of wearable technology) or other care pathway.
A regular day will see you:
- Meet with patients to assess their needs, and work collaborative with the patient, their carer and the referring unit to come up with an appropriate plan for home-based admitted care, then coordinate the services needed to make this happen.
- Review and assess patients to manage changing care needs or clinical deterioration, collaboratively with the nurse in the home or via virtual review.
- Collaborate with home team and specialty services to manage effective discharge planning to community based care.

- Post graduate in Cardiac/Crit care or other relevant post graduate certification preferred
- Previous hospital in the home experience, or experience in case management, liaison nurse, or nursing leadership position

Selection Criteria Essential: Registered Nurse registered with the Nursing and Midwifery Board of Australia
Essential: Current Victorian Driver’s license.
