**About your new role**

The Intake and Assessment Officer is part of a multidisciplinary mental health team, delivering high quality mental health intake, assessment, triage, and referral services for individuals in the community who experience mental health issues and are seeking support. The priority of the HtH PDIS is to connect people seamlessly to the most appropriate local service to meet their identified needs, consistent with the ‘stepped care’ approach.

Intake and Assessment Officers will provide an initial identification and assessment of individuals’ needs, as well as assist people requiring general information. The officer will have responsibility for undertaking an assessment using the Commonwealth’s Initial Assessment and Referral Decision Support Tool (IAR-DST), and work closely with the clinicians within the team to determine the level of clinical oversight, and key decisions required in matching people with the appropriate support service.

**What you will bring to the role?**
- Demonstrated experience in the provision of mental health intake, assessment and supporting consumers with presenting mental health concerns.
- Experience working with client information systems, maintaining accurate documentation and data collection.
- Proven ability to work autonomously and collaboratively within a team.
- Proven ability to maintain confidentiality and build trust to deal with sensitive and difficult situations in an empathetic manner.

**What the role requires**:

- National Criminal History Check (Police check) - disclosable outcomes considered
- Current Working with Children Check required before commencement of work
- Current Drivers licence
- Australian Working Rights
- As part of Neami’s condition of employment you will need to provide appropriate evidence of being fully vaccinated against COVID-19 (having received three doses) to Neami National.
