**About the opportunity**:
Due to increased consumer demand, we are seeking a **Support Coordinator** to service our consumers across **(Casey/Cardinia region) **to join a skilled and dedicated team who are committed to providing high quality services and go above and beyond for our consumers to coordinate and implement the support they need. In this full-time permanent role that is a mix of outreach, office based and remote work you will collaborate with individuals and families living with disability to strengthen their capacity and build resilience to be able to fully participate in the community.

Working from home with the ability to work from our Narre Warren and Pakenham offices, you will connect consumers with service providers to help them access new services and provide coaching and guidance to develop a plan and work towards their set goals.

**Desired Skills and Experience**:

- Relevant tertiary qualifications in disability or related discipline highly desireable
- 3 years’ experience within disability services or related services (i.e. Disability Employment Services);
- Demonstrated service knowledge of Disability, Mental Health, Family Violence, Alcohol and other Drugs;
- Demonstrated methods that promote engaging people with a disability into their community and a commitment to principles involved in community inclusion;
- Demonstrated time management in fast paced, complex service environment;
- Ability to use all relevant technology including tools enabling mobility and working remotely
