**Job summary**

BJ Disability and Support Services looking for experienced, reliable, and empathetic Support Workers (Eastern suburbs of Melbourne and surrounding areas) who want to make a difference by supporting our participants in achieving their goals and living their best life.

**About the role**

This key Client Liaison officer role is responsible for the day-to-day running of SIL homes, Community & In-home support participants.

The Client Liaison officer collaboratively works with the Branch Manager and support staff for optimal home outcomes and to support the clients to achieve their individualised goals and outcomes.

Our Client Liaison officer fulfils an important coordination, customer engagement, and employee management role for our business, and:

- is the primary contact for specific clients.
- is the line manager for all staff collaborating with the home’s clients.
- is responsible for the effective management of high-quality and individualised services being delivered to the client/s while ensuring the client/s retains choice and control.
- ensures all support services are delivered in line with the client's needs and personal goals,
- ensure a smooth transition into or out of the organisation as appropriate to a client’s situation.
- Development of professional relationships with the participants, their families, and other stakeholders and engagement and regular stakeholder meetings
- Maintaining participant's records and providing updates on progress in weekly, monthly, or quarterly reports
- Ensure the team complies with the Disability Services Act Standards, NDIS framework, and other relevant legislation.
- Proven experience implementing and updating support plans & assessments; for example, Positive behaviour support plans (PBSP), meal plans, Support plans, and Risk Assessments.
- Strong administrative skills in rostering, processing timesheets, and scheduling appointments using an online portal along with other general admin duties.
- Ability to be Oncall / Afterhours contact.

**Skills & Experience**
- Diploma or Certificate IV in Disability, Mental Health, Social Work or other relevant qualification
- Demonstrated experience of working in a leadership/management role
- Knowledge of trauma and its effects on the development and behaviors of people
- The ability to contribute positively within a team.
- NDIS knowledge and awareness
- A positive mindset, proactive can-do attitude, solution-focused
- Basic business understanding and the importance of professional attitude and meeting deadlines.
- Completion of NDIS mandatory training modules
- Working with Children Check
- Driver's License
- NDIS Worker Screening Check

**Job Types**: Part-time, Casual
Part-time hours: 25 per week

**Salary**: $35.00 - $40.00 per hour
