**About the Program**

The Women Children and Family Health Service aims to support safe nurturing environments for pregnant women, infants and children, increase uptake and utilisation of services with an emphasis on early intervention and prevention, provide streamlined coordinated care and positive experiences for clients to encourage continued engagement with services.

**About the Role**

Working under limited direction of the Team Manager, Maternal and Child Health, the primary role of the Aboriginal Child and Family Linker (Case Management) is to support parents/caregivers access the services they need for their children, including and not limited to:

- Build trust and connection and maintain strong and respectful relationships with parents/caregivers of Aboriginal children who have developmental vulnerabilities
- Empower parents/caregivers and increase their capacity to support their children’s learning and development, including access to quality information and resources
- Based on family strengths and needs work alongside parents/caregivers to connect them to resources and support they need
- Manage all service referrals, appointments and engagement for parents/caregivers and children to Kudos Services (NDIS partner in South Australia for the Early Childhood)
- Promote and support positive relationships between families and early childhood services including developmental opportunities in the community (e.g. playgroups, childcare, early learning centres)
- Develop and maintain a strong and broad network with local service providers and use relationships to leverage priority access for families to the supports they need
- Support access to and integration of early childhood services for Aboriginal children at risk of developmental vulnerability
- Manage all administrative tasks required of the role including clinical and referral documentation and appointment management
- Participate in service training, continuous quality improvement and evaluation activities.

**Remuneration**

This position is under the Nunkuwarrin Yunti Enterprise Agreement classification of **Health Services** Level **4 **(Salary sacrifice and paid Christmas and New Year break)

**Desired Skills and Experience**:
All positions within Nunkuwarrin Yunti of SA Inc will be required to undergo a six (6) months probationary period.

**ESSENTIAL - includes qualifications, skills, experience and knowledge.**
- Minimum qualifications Certificate IV in Aboriginal Primary Health Care or a recognised qualification in social work, early childhood education and care, behavioural science or related field deemed an equivalent level and expertise to undertake the range of activities required of the role
- May hold registration with AHPRA or registered as a member of an appropriate professional body (e.g. AASW)
- Minimum three (3) to four (4) years demonstrated vocation experience in field relevant to the role
- Sound knowledge of the issues affecting Aboriginal and/or Torres Straits Islander families and an ability to communicate sensitively and effectively with Aboriginal and / of Torres Strait Islander people
- Ability to deal with people with high and complex needs and the ability to deal with conflict constructively
- Ability to liaise, network and negotiate with a range of human service organisations and government agencies.
- Well-developed skills with a general knowledge of child health and development and family health
- Sound knowledge of best practice approaches to comprehensive primary health care
- Ability to participate in the development and coordination of care plans
- Demonstrated experience in managing a diverse range of tasks and competing priorities, working with mínimal supervision and working as a member of a team.
