**About the Program**

The Women Children Family Health Service aims to support safe nurturing environments for pregnant women, infants and children, increase uptake and utilisation of services with an emphasis on early intervention and prevention, provide streamlined coordinated care and positive experiences for clients to encourage continued engagement with services.

Working under limited direction of the Projects Manager, Maternal Child and Family Health, the primary role of the Aboriginal Maternal and Child Health Linker is to support a small caseload of pregnant women and the parents/caregivers of young Aboriginal children (0-4 years) to access the services they need for their health and wellbeing.

**About the Role**

The primary role of the Aboriginal Maternal and Child Health Linker is and not limited to:

- Build trust and strong respectful relationships with pregnant women and parents/caregivers of Aboriginal children.
- Walk alongside pregnant women and parents/caregivers to identify support, priorities and preferences to meet their health and wellbeing needs.
- Support pregnant women and parents/caregivers to understand health information and confidently access appropriate services, providing a bridge between families and their health service providers and across health care settings.
- Identify barriers to care by identifying critical resources and supports to facilitate continuity of care.
- Coordinate appointments, communications with health providers, and follow up of referral information and documentation.
- Follow up families who may have disengaged from their care, explore barriers and assist re-engagement in services as appropriate.
- Develop and maintain a strong and broad network with local service providers and use relationships to leverage priority access for families to the supports they need.
- Participate in service training, continuous quality improvement and evaluation activities.

**Remuneration**

This position is under the Nunkuwarrin Yunti Enterprise Agreement classification of **Health Services** Level **4 **(Salary sacrifice and paid Christmas and New Year break)

**Desired Skills and Experience**:
All positions within Nunkuwarrin Yunti of SA Inc will be required to undergo a six (6) months probationary period.

**ESSENTIAL - includes qualifications, skills, experience and knowledge.**
- Certificate IV in Aboriginal Primary Health Care (Practice) or a recognised qualification in social work, early childhood education and care, behavioural science or related field deemed an equivalent level and expertise to undertake the range of activities required of the role.
- Holds registration with AHPRA or registered as a member of an appropriate professional body (e.g. AASW).
- Minimum three to four years demonstrated vocation experience in maternal, child and/or family health.
- Sound knowledge of the issues affecting Aboriginal and / or Torres Strait Islander families and an ability to communicate sensitively and effectively with Aboriginal and / or Torres Strait Islander people
- Ability to deal with people with high and complex needs and the ability to deal with conflict constructively.
- Ability to liaise, network and negotiate with a range of human service organisations and government agencies.
- Ability to participate in the development and coordination of care plans.
- Demonstrated experience in managing a diverse range of tasks and competing priorities, working with mínimal supervision and working as a member of a team.
- Demonstrated ability to undertake various health work activities requiring a high level of knowledge and skills, including quality improvement activities.
- Experience in the use of computer software, especially Patient Information Recall Systems (e.g. Communicare, Medical Director) and other databases.
- Ability to use professional judgement to prioritise care and organise workloads.
