**About the Program**

The Women Children Family Health Service aims to support safe nurturing environments for pregnant women, infants and children, increase uptake and utilisation of services with an emphasis on early intervention and prevention, provide streamlined coordinated care and positive experiences for clients to encourage continued engagement with services.

Working under limited direction of the Projects Manager, Maternal Child and Family Health, the primary role of the Service Navigator, Aboriginal Maternal and Child Health, is to support development and testing of an Aboriginal Maternal Child Health Linker Program within an ACCHO setting to facilitate continuity of care for mothers of Aboriginal babies and Aboriginal children aged 0-4 years.

**About the Role**

The primary role of the Service Navigator, Aboriginal Maternal child and Family Health is and not limited to:

- Lead care coordination for a small caseload of clients/families with complex needs to support continuity of care across their health care journey, including support and advice for other members of the health care team
- Provide senior level input and advice to the design, testing, data collection and analysis of Project activities, including suitability of MCH Continuity of Care Protocols
- Negotiate with external agencies with emphasis on identifying and addressing barriers of access to secondary and tertiary services
- Document client/family experiences of success and/or factors contributing to dis-continuity of care, and possible solutions that might be helpful for other clients/families
- Identify and document service and system gaps that impact on continuity of care with emphasis on interface between health care settings
- Participate in and where appropriate provide high level advice for SA Health MCH Continuity of Care Project Working Groups
- Ensure all service delivery meets legislative requirements, professional standards and complies with funding contract expectations
- Contribute to data and information collection and provide reports, analysis and interpretation for senior management

**Remuneration**

This position is under the Nunkuwarrin Yunti Enterprise Agreement classification of **XXXXXXXX** Level **XXXXX **(Salary sacrifice and paid Christmas and New Year break)

**Desired Skills and Experience**:
All positions within Nunkuwarrin Yunti of SA Inc will be required to undergo a six (6) months probationary period.

**ESSENTIAL - includes qualifications, skills, experience and knowledge.**
- Registered with Australian Health Practitioner Registration Authority (AHPRA) or another suitable nationally recognised professional association
- Demonstrated knowledge and skills commensurate with the level of autonomy, decision making authority and influence
- May hold a Medicare Provider Number
- Demonstrated knowledge and understanding of Aboriginal and Torres Strait Islander societies and culture and awareness of issues which may impact on maternal child and family wellbeing
- Demonstrated ability to communicate sensitively and effectively with Aboriginal and Torres Strait Islander people
- Significant experience working within an Aboriginal Community Controlled Health Service or Primary Health Care setting including maternal, infant, child and/or family health
- Demonstrated knowledge of the healthcare system and strong relationships with local key health care services and providers
- Significant experience to liaise, network and negotiate with a range of human service organisations and government agencies.
- Demonstrated experience in managing a complex range of tasks and competing priorities, working with limited supervision in a cross-sectoral environment
- Experience in the use of computer software, especially Patient Information Recall Systems (e.g., Communicare, Medical Director) and other databases
- Ability to use professional judgement to manage own and other workloads, prioritise care needs and resolve issu
