**About the Role**:
Join our team as a Advance Care Planning Facilitator on a fixed term (9 weeks), based at Monash Medical Centre Clayton

Advance care planning supports person centred health care, ensuring that a person’s preferences are known and utilised to inform the provision of health care when a person is unable to participate in decisions about their care due to a loss of decision making capacity.

The primary role for this fixed term position as advance care planning facilitator is to:

- Support our consumers to discuss, plan and formally document their future health care preferences.
- Develop advance care plans that are clear and concise and individual to the client and comply with legislative requirements
- Receive, evaluate and process referrals to the Shared Decision Making / Advance Care Planning Office
- Consultation and support to Monash Health employees across a variety of health care settings
- Actively promote shared decision making and advance care planning within Monash Health and the broader community

**About Us**:
The Shared Decision Making / Advance Care Planning Office supports autonomy and self-determination for consumer for delivery of person-centred care. The Office has several key tasks and responsibilities aimed at supporting and embedding and sustainable approach for shared decision making and advance care planning across the organisation. Key tasks include the provision of clinical service to consumers to plan and document future health care preferences; education, training, mentoring and consultancy to a range of health care professionals; policy and procedural development, and utilisation of best practice information, standards and frameworks to develop, support and evaluate an organisational wide shared decision making approach to care.

**About You**:
To ensure your success in this role you will bring the following demonstrated skills, experience and knowledge:

- Highly developed interpersonal skills and communication skills, both verbal and written
- Demonstrated proficiency in comprehensive psychosocial assessments, intervention, advocacy and counselling
- Work effectively within a culturally responsive framework sensitive to specific needs of individuals; ability to communicate information in a meaningful way free from jargon, regardless of language, culture, age, class, gender
- Ability to work independently and as part of a team
- Demonstrates advanced awareness of social, political, legal, cultural, and organisational contexts and systems and how they impact on people.
- Demonstrate an advanced level of knowledge and expertise relevant to advance care planning and the Medical Treatment Planning and Decisions Act (2016) and the Voluntary Assisted Dying Act (2017)
- A sound understanding and expert ability in navigating complex service systems
- Experience in working in health care across the care continuum
- Confidence in working with multidisciplinary teams to support consumers with comprehensive future care planning
- Good organisational skills and effective time management
- Experience in introducing and completing Advance Care Plans with consumers
- Working with adult consumers (and their significant others) living with a life limiting illness
- Computer literacy with Microsoft office, excel

Mandatory requirements of the role - qualifications, licences, memberships,
- Normally have at least 7 years’ post qualifying experience
- Written evidence of eligibility for AASW membership for overseas qualifications is required.
- Current Victorian Drivers Licence (or equivalent) essential and use of own care is required (Travel will be reimbursed as per Monash Health Private Vehicle Travel Allowance Policy)
