Sil Care Coordinator

Direct Care Assistance (DCA), a leading NDIS Registered Service Provider, is on the lookout for a dedicated SIL Care Coordinator to enhance our team. Our commitment lies in delivering top-notch support to our NDIS participants while upholding the highest industry standards.
**Role Overview**:
In this vital position, you'll be essential in aiding our valued clients, particularly focusing on participants in Supported Independent Living (SIL) settings with various disabilities and complexities. Your main duties will involve overseeing these clients' care, care plans and collaborating closely with their support networks and care team.
**Key Responsibilities**:
- Efficiently communicate with the participant's support team including the support coordinator, guardians, advocates, and family members as well as with DCA operations manager.
- Conduct regular meetings with the care team to review and implement the care agenda effectively.
- Coordinate with the Rostering and HR departments to address support worker issues or recruitment needs.
- Monitor and ensure the delivery of high-quality care by reviewing participant's report, care plans and addressing any issues promptly.
- Confirm that all support workers fully understand and can meet the participants' needs in line with DCA's values.
- Report directly to the operations manager and work collaboratively.
- Perform regular SIL property inspections and address any identified issues.
- Maintain accurate participant and team records in DCA's record system.
- Handle the onboarding process for new clients, including initial assessments and compliance documentation.
- Engage with participants, their networks, and other stakeholders to streamline the transition process.
- Collaborate with the Direct Care Assistance team (HR, finance, operation manager and Rostering) to deliver top notch care to participants.
- Ensure accurate and updated data management of participants.
- Liaise with the recruitment team to find suitable support workers for unique client care needs.
- Review support worker performance and coordinate with HR for any necessary actions.
**Requirements**:
- Must have qualifications in disability services.
- Proven experience in the disability sector, specifically within NDIS services and SIL settings.
- Understanding of NDIS guidelines and support structures.
- Knowledge of relevant legislation and safety regulations.
- Strong communication and team engagement abilities.
- Excellent organizational skills and meticulous attention to detail.
- Ability to work both independently and collaboratively.
- Commitment to upholding the rights and dignity of individuals with disabilities.
- Computer proficiency, especially in Microsoft Office.
- Necessary certifications.
