The Health Promotion Coordinator will lead and manage the health promotion activities for NRCH under the service and funding agreement which includes the development and operationalisation of a 4-year Community Health Health Promotion Plan and an annual action plan to prioritise action that reflects state and national health priorities.

To work in a collaborative manner within NRCH and the wider community, to implement effective health promotion interventions and capacity building strategies.

The Health Promotion Coordinator will be responsible for Identifying and addressing priority health and wellbeing issues within the local government area with a population health focus on reducing health inequities.

- Improving mental health and wellbeing
- Improving healthy eating and active living
- Reconciliation
- Gender Equity
- Climate Action
- Diversity
- Chronic Disease

The Health Promotion Coordinator will be familiar with population health data and identify health promotion and prevention needs of our community. The Coordinator will ensure services are delivered in line with NRCHs Strategic Plan and the Department of Health CHHP Guidelines.

**Roles and responsibilities include, but are not limited to**:

- Development and delivery of annual CHHP action plans and when necessary, a 4-year strategic plan, meeting all requirements set by Department of Health and the satisfaction of NRCH, including identify intervention types and capacity building strategies.
- Meeting service and funding agreement targets and workplan service levels, including reporting requirements.
- Timely annual reporting, meeting NRCH management satisfaction and the requirements under the service and funding agreement.
- Lead and support campaigns and collateral development relevant to the CHHP Plan priorities.
- Achievement of the goals and outcomes specified in the CHHP Plan.
- Documentation of Health Promotion workforce development plan.
- Coordinates the implementation to achieve the planned outcome measures of the CHHP Plan in consultation with the Department of Health, other managers and direct reports.
- Develops and builds health promoting capacity of NRCH in collaboration with key community and agency stakeholders.
- Applies the Social Model of Health, Population Health, CHHP guidelines and various other frameworks to guide program planning, provision and evaluation.
- Driving strategies that direct NRCH to be a health promoting organisation committed to promoting the health of our staff, clients and the broader community.
- Maintain strong client and stakeholder management that reflects partnership building to strengthen the reach and scale of health promotion messages.
- Drive and support the NRCH Reconciliation Action Plan.
- Represents NRCH in CHHP at an organisational, local and state level as required.
- Maintains robust documentation of CHHP programs.

**Key Selection Criteria**

**Essential qualifications, attributes and skills include, but are not limited to**:

- Minimum 3 years’ experience working in health setting as Health Promotion practitioner
- Bachelor Degree of Health Science with a major in Health Promotion, or Bachelor Degree of Health Promotion, Public Health or Population Health, or tertiary qualification in a health-related field.
- Sound knowledge of health promotion theory and practice
- Commitment to improving the health of vulnerable and marginalised communities
- Demonstrated leadership and interpersonal skills
- Demonstrated ability to use research to support best pract
