Case Manager

We are seeking a Case Manager with experience in community aged care, and familiar with the commonwealth home care packages program. The organisation is an established and highly respected not for profit, national community care provider. The service is aimed at ensuring people are supported and set to achieve their individual goals while living at home.
As a member of this forward thinking and innovative team you must be committed to client focused care, able to assist in the planning, implementation and evaluation of consumer care plans. In this important position you will lead by example and share knowledge and keep abreast of changes and developments in regulations and legislation within your areas of responsibility.
You must be able work well in a team and have a passion for individual care and treat colleagues and consumers with respect and dignity in line with the values of the organisation.
On offer is an attractive salary and the benefits of salary packaging, km based travel allowance.
**Case Manager Profile**:
- Previous Case Manager experience
- Sound knowledge and able to monitor costs and funded budget
- Ability to work autonomously and also in a team environment
- Excellent knowledge of Aged Care, OHS, Home and Community Care and other relevant legislation
- Great communication skills, able to build relationships with consumers, colleagues and with other external healthcare professionals
- Provide leadership and support to direct care employees
- Strong critical thinking and decision-making skills
- Current National Police Check
- Must have valid Victorian Driver’s Licence and own car
