**MAIN DUTIES/RESPONSIBILITIES**:

- Supporting clients to develop their capacity, while supporting their understanding of the NDIS and empowering/supporting them to make decisions and exercise control over the goals they have set for themselves via an NDIS plan..
- Coordinating a range of services/solutions/supports from several providers to meet the demands of the client.
- Coordinating client plans to enable greater independence across all domains of life.
- Working within each person's funding levels (continually track, monitor, and report funding budgets).
- Compiling and documenting relevant information about clients from other services for the development of evidence-based reports and updates.
- Communicate and collaborate with key stakeholders to establish a common goal of coordinating service delivery and developing effective relationships relevant to the achievement of NDIS goals.

- You may be required to undertake tasks beyond those stated in the job description to satisfy our operational and commercial needs.
- Your a job may need you to travel on short notice.

- Commitment to the concepts of quality assurance and continual improvement
- Encourages a culture of continuous quality improvement through discussion and collaboration.
- Identify improvement possibilities, ideas, and suggestions for quality enhancements.

**Skills**:

- A tertiary qualification in allied health/social work, membership to a relevant national accreditation body, and experience in NDIS Support Coordination are required
- Excellent problem solving, financial management, and report writing skills
- A demonstrated ability to adapt to change and to display initiative in relation to continuous improvement
- Basic financial literacy required for understanding NDIS funding and analysis.
- Possess strong verbal and written communication skills
- Demonstrate patience and empathy.
- Have Demonstrate fantastic people skills
- Be proactive, adaptable, and able to work autonomously
- Demonstrate excellent negotiation and networking skills
- Have a valid Victorian driver's license
- The successful applicant will need to get an NDIS Worker Screening Check (at your own cost)

**You will need**:

- 100 Points of identification.
- Have a valid driver's license and a fully insured vehicle.
- Completed NDIS Worker Screening Check.
- Working with children check.
- Full Australian work rights.

**Job Type**: Casual

**Salary**: $28.61 - $60.01 per hour

Schedule:

- 8 hour shift

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
