**ROLE OVERVIEW**:
**KEY RESPONSIBILITIES**:

- Assess the needs of individuals with disabilities to develop personalised care and support plans.
- Coordinate and oversee the delivery of a range of services, including therapeutic support, community participation, household tasks, and travel/transport assistance.
- Actively engage with participants and their families to ensure services are delivered in alignment with their goals and preferences.
- Work collaboratively with healthcare professionals, support workers, and other stakeholders to achieve optimal outcomes for participants.
- Monitor and review the effectiveness of support services, making adjustments as necessary to meet changing needs.
- Facilitate innovative community participation initiatives to encourage social connection and active engagement of participants in the community.
- Ensure compliance with all relevant regulations and standards pertaining to disability care and support.
- Provide ongoing support and advice to participants, empowering them to achieve greater independence and quality of life.

**Requirements**:

- Australian Citizens and Permanent residents are encouraged to apply.
- Minimum Diploma in social work, community services, disability services, or a related field.
- Minimum 2-3 years of experience in disability care, community services, or a similar role.
- Strong understanding of the challenges faced by individuals with disabilities and a passion for enhancing their quality of life.
- Excellent interpersonal and communication skills, with the ability to work effectively with participants, families, and team members.
- Demonstrated ability to develop and implement effective care and support plans.
- Commitment to upholding the values and mission of Westview Disability Care.
