About the opportunity
Working in partnership with clients and stakeholders, the Care Coordinator ensures that services are allocated in a timely manner and with mínimal disruption to client preferences and workforce availability.
Key Responsilbitites

**You will thrive in this role as you**:
Demonstrate consideration, understanding and respect for clients and their families at all times in all interactions.
Ensure personal and team contribution and support overall team effectiveness by demonstrating a high level of commitment and efficient follow through of any tasks until completion or as otherwise agreed with the Manager of Care Coordination and Quality.
Attend to incoming calls from Support Workers and clients with scheduling and service related enquiries.
Action all service requests in a timely manner.
Prepare and distribute fortnightly rosters to Support Workers efficiently.
Validate Support Worker hours worked.
Arrange transport and Support Workers for client excursions.
Record all changes in Support Workers availability.
Monitor Support Workers Annual Leave.
Confirm shifts.
Attend to feedback and reports from Support Workers in relation to clients.
Assist in coordination of Support Workers monthly meetings and training.
Monitor and report staff shortage.
Advise clients and Support Workers in a timely manner of changes to care and service delivery.
Liaise with the accounts team in relation to rostering/payroll discrepancies.
On call allocation as per the team roster.
Participate in the orientation of new staff.
Meeting minute taker as required.
Maintain filing of documentation and general admin tasks.
Be responsible for implementing and integrating relevant written policies and procedures into your day-to-day work practice and to be aware that a breach of the confidentiality and privacy of critical documents may result in disciplinary action.
Other duties as delegated by the Manager, which may also relate to general organisational demand.
To ensure accurate case notes for every schedule change noting the reason for the change and who initiated the change.
Assist with communication to Support Workers for specific instructions on services.
To accurately record all episodes of being unable to fill a service.
To ensure Support Worker's hours are rostered so as not to exceed 38 hour per week.
Key Selection Criteria
Minimum of year of experience coordinating rostering services.
Home Care or Disability services industry expertise (preferred).
A working knowledge of the Aged Care Quality Standards commensurate to the role of the Care Coordinator.
Highly developed data input skills.
Highly developed prioritisation skills with a proven ability to manage work load in order to achieve business outcomes.
Highly developed software literacy skills across all common MS Office products and specifically in rostering software packages.
Well developed problem solving skills.
Well developed written and verbal communication skills, including negotiation skills.
