**Role**:
The Disability Inclusion Coordinator will have management responsibility and accountability for the implementation of the DET Disability Inclusion reform. They will report directly to the Leading Teacher Student Inclusion and the Assistant Principal. The Disability Inclusion Coordinator will be required to manage education support staff in achieving specific objectives in relation to funded and non-funded students with disabilities, as well as working with teaching staff, external services and DET personnel to guide and inform educational programs at the College. It is anticipated that this role will include classroom learning support for funded and non-funded students.

**Responsibilities**:
Key responsibilities of this position will include:
**Program for Students with Disabilities (PSD) & Individual Educational Plans**
- Oversight and management of the PSD program including the development and implementation of the Inclusive Education Support staff timetables and allocation of students
- Convene and record Inclusive Education Support staff meetings
- Facilitation of Student Support Group Meetings including reviewing and developing Individual Learning Plans (ILPs) and liaising with teachers to ensure goals are being met
- Management and monitoring of the phasing out of the PSD program including the transition of funded students to Tier 2 & 3 funding
- Oversight of Individual Education Plans (IEPs) across the school and ensuring that the school meets its obligations for students who are required to have an IEP
- Ensuring that students have a voice in setting learning targets during the IEP process
- Promotion a culture of high expectations and aspirations for the learning of students with disabilities or additional needs
- Acting as the main contact for families of students with disabilities and additional needs

**Disability Inclusion Process Responsibilities**
- Being the lead contact for the Disability Inclusion students within Point Cook Senior Secondary College
- Attend Disability Inclusion Professional Development in conjunction with the Leading Teacher Student Inclusion
- Coordination of a staged approach for Disability Inclusion Profile including managing timelines
- Collection of the required evidence for DIPs including Vineland-3 Assessments along with other required supporting evidence
- Working with teachers, coordination of the implementation of 10 weeks of adjustments and interventions to support the DIP process
- Coordination and recording of outcomes of Disability Inclusion Profile Meetings with DET staff
- Provision of additional information in keeping with legislative requirements in response to requests from facilitators and external providers
- Ensuring that Disability Inclusion Profile meetings are systematically held for all students at Tier 3 and those students at Tier 2 as prioritised with Leading Teacher Student Inclusion, Year Level Leaders, and Wellbeing Team members
- Ensuring that all requirements are met for the Disability Inclusion Profile meetings for students at Tiers 2 and 3
- Facilitation of Student Support Group meetings for students at Tier 3 and oversight of the management of SSGs for Tier 2 students in consultation with the Leading Teacher Student Inclusion as required
- Supporting school staff and gathering all supporting information for Disability Inclusion profile meetings
- Liaison with the school’s Health and Wellbeing Key Contact (HAWKC) and Student Support Services key contacts as required.
- Making class observations for Tier 3 and then Tier 2 students, to help address the learning needs of these students

**Professional Development**
- Undertaking classroom observations to support teachers in the provision of curriculum and appropriate learning activities for Tier 2 and Tier 3 students
