**About the role**

The assessor is responsible for providing up to date information on appropriate aged care pathways that the client may be eligible to access. These include, but are not limited to community, permanent residential, residential respite and flexible care options and both State and Commonwealth funded pathways available under the _Aged Care Act 1997_. As an assessor, you will liaise with clients, carers, other health professionals, service providers and other internal and external stakeholders.

As an assessor, you will ensure that you work within the Aged Care Assessment Programme guidelines and met assessor benchmarks and Key Performance Indicators (KPIs), as set out by the Commonwealth government and the Office for Ageing Well (OFAW) - SA Health. You will participate in education, administration, evaluation and quality assurance activities and ensure the legislative and funding requirements of the Department of Health and Ageing are met.

**Essential Requirements**:

- Current National Police Clearance (NPC) less than 12 months old
- Current Aged Care Screening check
- Immunisation screening as indicated in the role description. SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.
